Add it to your oil 1000 miles or so before a change is due and it will give your engine a thorough cleaning!
Drop 1/2 can in the oil and the other half on the gas tank when you go to fill up.
Seafoam is the brand name. You can get it at places like Autozone and other car places.
Regarding the hose, it's no biggie, it won't affect your oil pressure and you will probably only lose minimal amount of oil if any. I wouldn't worry about it unless it is seeping onto the floor or causing a mess.
Ill try and find a better place to buy that hose from and get back to you if i find it. May not cost all that much through the dealer though.

I recommend slow cleaning process, i.e. use synthetic oil and fuel system cleaner. If you have sludge in your engine and seaform dissolves it right away. You'll be in a big trouble. Remember oldschool method of adding ATF before changing oil, seafoam does the same thing. Just my .02
